The Polyvagal Ladder
Dr. Stephen Porges' Polyvagal Theory describes three distinct states of the autonomic nervous system. Think of them as rungs on a ladder — we move up and down depending on what our nervous system perceives as safe or threatening.
Neuroception of Safety
Rest & DigestVentral VagalWhen your nervous system detects safety, you feel connected, calm, and present. You can think clearly, learn, relate to others, and access your full self.
- Feeling grounded and present
- Able to connect with others
- Open to learning and new experiences
- Regulated breathing and heart rate
Neuroception of Danger
Fight or FlightSympathetic NSWhen your nervous system detects danger, it mobilizes you for action. Blood and energy are re-routed away from digestion and thinking toward survival. This is the state first responders operate in on every call.
- Increased heart rate and adrenaline
- Thinking brain goes 'offline'
- Non-essential body functions pause or stop
- Irritability, anxiety, or hypervigilance off-duty
Neuroception of Life Threat
Freeze / CollapseDorsal VagalWhen the nervous system perceives an inescapable threat, it shuts down to conserve energy. This can look like numbness, dissociation, emotional flatness, or feeling completely disconnected from life.
- Emotional numbness or shutdown
- Dissociation or feeling 'not here'
- Extreme fatigue or collapse
- Release of endogenous opioids (natural pain suppression)
What This Means for You
Your nervous system is not broken — it is doing exactly what it was designed to do. The challenge for first responders is that years of repeated activation without recovery can leave the nervous system stuck in danger or collapse states even when you are off duty and objectively safe.
The goal of therapy is not to eliminate these responses — they keep you alive on the job. The goal is to build flexibility: the ability to move back up the ladder toward safety and connection when the threat has passed.
Noticing which rung you are on right now is the first step. You cannot regulate what you cannot recognize.
